Electronic Medical Records (EMR) System
Electronic Medical Record (EMR) systems play an important role in ensuring continuity of case, particularly in settings where case managers, clinicians, and health workers need to track patient journeys across multiple visits, diagnoses, and interventions.
Create your own database based on this template
This template uses the Record Detail View, which provides a powerful way to bring together all relevant patient information onto a single, organized page. This eliminates the need to switch repeatedly between records and sub-records, streamlining case management and improving decision making.
Important warning: This is a template, not a finished system
This Electronic Medical Records (EMR) template is for training and demonstration purposes only and serves as a starting point. It is not a ready-to-use, clinically validated system.
Before using this template with real patient data, you must consult with qualified healthcare professionals to customize the forms and workflows to match your specific clinical needs and ensure compliance with all local and national data privacy and healthcare regulations.
Failure to properly design, configure, and validate your EMR system can pose serious risks to patient care and data privacy.
In ActivityInfo, the Record Detail View transforms a typical Electronic Medical Record (EMR) workflow into an interactive, patient-centered experience. Instead of opening a parent form and multiple subforms for patient registration, triage, consultation visits, lab results, imaging results, treatments, procedures and finance, all this information can be consolidated onto one page for each patient. The following are the steps to follow to create a Record Detail View:
Form design
The template consists of a number of forms.
The Patient Registration (Main Form) acts as the parent record for each patient.
Fields include:
- Patient ID (Serial Number)
- First & Last Name, Date of Birth, Gender
- Contact Information, Allergies, Chronic Conditions
This registration form in turn has a number of subforms:
- Triage: Captures initial clinical assessment, with fields including the reason for visit and vital signs
- Consultation Visits: Each entry documents a clinician encounter with fields including: Visit Date, Clinician Name, Diagnosis, Notes
- Lab Tests
- Imaging Diagnostics: Records Imaging Date, Imaging Type (X-ray, CT Scan, MRI, Ultrasound, Image attachments)
- Prescriptions: Captures medications prescribed.
- Procedures: Records clinical or surgical procedures.
Record Detail View
The Record Detail View in ActivityInfo provides a consolidated view of details related to a single record, presented on one page. This is particularly useful in workflows such as case management, grant management, project management, electronic medical records review among other use cases where users need to view both high-level information and detailed sub-records at the same time.
Instead of switching back and forth between forms and subforms, users can review all critical information and related interaction in one place. Some of the challenges addressed by this functionality include fragmented information where data stored in multiple subforms requires repeated navigation to piece together a parent record and the related sub-records.
This also helps in easing the reporting burden where users need to connect multiple datasets manually, increasing the risk of oversight or errors.
