Many ActivityInfo users are working in settings where they need to collect information related to COVID-19 as a result of the Coronavirus disease (COVID-19) outbreak situation. To make your work easier we designed some suggested Form templates with which you can quickly start collecting data. You can copy these sample Forms and adjust them to your needs. You can also import the Form Schemas or specific fields of the Schemas to get quickly started. Alternatively, you may use these templates to get some inspiration for your own Forms.

In each case we suggest adding a Reference Form to include all the medical institutions supporting the COVID-19 response in your country or the area for which you are collecting data. This will allow you to collect all the medical institutions and their geographical location in one place and make sure that all users reporting use the same names for the medical institutions.

The following Form templates are suggested samples with dummy information and not official Forms of any organization. You can find the respective templates and step-by-step instructions as well as the Form Schemas in the User Manual articles listed with each template below.

We welcome all organizations and users who want to share their own templates and Forms for other organizations to copy or take inspiration from, to contact us. We will make sure to add these templates to our User Manual as well.

COVID-19 - Testing - tests performed and results

This is a template that can be used to collect information about the number of tests performed per day in your country along with the number of tests with a positive result and the number of tests with a negative result for COVID-19. The data can be linked to the medical institutions of the country. You can add more fields to collect disaggregated data such as the number of tests performed by gender, the number of positive test results by gender and the number of negative results by gender.

The template includes a Form with a Subform. The Form links to the Reference Form to allow users to select the institution for which data is collected from a list of institutions. The Subform collects the number of tests performed along with the number of positive and negative results and the ratios of each type of result to the total number of tests performed.

COVID-19 - Test capacity

This is a sample template of a Form that can be used to collect data about the test capacity for COVID-19 for each medical institution in a country. The test capacity depends on people (e.g. personnel to conduct the test, to analyze the results, to deliver the results) and material (e.g. protective equipment, laboratory equipment, etc.) available in each medical institution. The Form can be used to collect daily data about the availability and shortages of such resources.

The template includes a Form with a Subform. The Form links to the Reference Form to allow users to select the institution for which data is collected from a list of institutions. it also uses Calculated fields and Formulas to automatically sum up or get maximums and averages of the numbers reported in the Subform.

COVID-19 - New and Total cases, deaths and recovered

This is a template that can be used to collect information about the numbers of new COVID-19 cases, new deaths and new recovered cases per day. The Form automatically calculates the total numbers for these per day. This template is handy if multiple users from different institutions report at different times of the day, for the same day.

The template uses a Form and a Subform. The Form can be used to collect daily data and links to the Reference Form to allow users to select the institution for which data is collected from a list of institutions.

The Subform collects the numbers of new affected cases, new deaths and new recovered cases. With Calculated fields in the Parent Form we automatically sum up the numbers reported in the Subform and we get the total number of affected cases, deaths and recovered cases per day.

COVID-19 - ICUs availability remaining

This is a template that can be used to track the remaining ICUs per medical institution. To be able to track the remaining units you need to know the initial total ICUs for each medical institution. Then, for each day you need to collect the number of ICUs that become available and the number of ICUs that are newly occupied on that day.

The template uses a Form and a Subform. The Form links to the Reference Form to allow users to select the institution for which data is collected from a list of institutions. It also captures the initial amount of available ICUs per institution.

The Subform can be used to collect daily numbers of ICUs that become newly occupied and the number of ICUs that become newly available, on that day. With a Calculated field the Parent Form calculates the remaining ICUs based on the numbers reported in the Subform.

COVID-19 - Individual level tracking

This is a template that can be used to collect information at an individual level. You may want to track the progress of the health of an individual patient affected by COVID-19. In that case, you may want to collect information regarding the medical institution where they have been hospitalized or tested, the dates when they were tested for the COVID-19 and the results of the tests as well as the isolation period they had to go through. At the same time, you may need to keep the individual anonymous.

The template uses a Form and a Subform. The Form links to the Reference Form to allow users to select the institution for which data is collected from a list of institutions. It uses as Key a Serial Number field to anonymize the individual. With the Form you can also capture the Date of hospitalization and automatically get the minimum required date for isolation on the basis of 16 days.

The Subform can be used to collect information about the dates of the user’s tests and their results. Users can add one Record in the Subform for every test taken by the individual.

COVID-19 - Individual cases spread

This is a Form that can be used to collect information at an individual level. You may need to collect data to track an individual patient along with the people they came in contact with while they were infected with COVID-19 in order to contact them. You may also need to collect data regarding the places the individual has travelled to. At the same time you may need to keep the individual anonymous.

The template uses a Form and two Subforms. The Form links to the Reference Form to allow users to select the institution for which data is collected from a list of institutions. It uses as Key a Serial Number field to anonymize the individual. With the Form you can collect information such as the day the individual was tested, whether the individual was in contact with other people or travelled 15 days before or after the testing. The number of days can be changed according to your needs.

The first Subform can be used to collect information regarding the travels the individual has made. Users will add one Record to the Subform for each travel. The second Subform can be used to collect information regarding the people the individual was in contact with during the defined period. Users will add one Record to the Subform for each contact.
